In this week’s #MedicalMinute, we’re highlighting how Kaleida Health is helping moms/birthing persons stay healthy and supported after delivery through the Post Birth Alert initiative. This program ensures new mothers/birthing persons receive timely follow-up care by connecting hospitals, OB-GYNs and primary care providers throughout the community.
As part of the initiative, moms/birthing persons are discharged from the hospital wearing a bracelet that identifies their recent birth status. This small but meaningful step helps providers recognize postpartum patients and allows them to offer the right care at the right time. What seems like a simple accessory is already making a big impact across Western New York – improving outcomes and giving families added peace of mind during those first important weeks at home.
